This new offering integrates clinical support, wellness services, and contextual navigation while utilizing AI to deeply analyze comprehensive data sets for each patient. By doing so, ReallyWell creates a unique, one-size-fits-one engagement experience. It delivers clinically prioritized wellness journeys that guide members step-by-step toward better health through tailored content and recommendations that adapt dynamically as a patient’s health evolves, delivering significant value.

In fact, ReallyWell is the first platform to seamlessly combine clinical care, wellness services, and contextual navigation across the entire continuum of care. With one-click omnichannel access to a nurse or health coach, members can quickly find the right resources or care protocols for any question or medical concern. This immediate connection to holistic care helps solve the navigation challenges that traditionally exist in a fragmented healthcare system. Additionally, it promotes proactive health management through early detection and timely intervention when new medical issues arise.

The need for this type of solution is clear. On average, users of wellness apps spend only two to three minutes per day on them, and more than half uninstall the apps within 30 days of downloading. In contrast, ReallyWell has shown impressive results, achieving an 83% gap closure rate for digitally opted-in wellness exams and an 86% sustained engagement rate.

At the heart of ReallyWell is its Wellbeing Copilot, which consolidates and synchronizes each member’s wellbeing and healthcare interactions, claims, advanced biometrics, clinical risk factors, social determinants of health (SDOH), behavioral needs, and more. Powered by generative AI, the Wellbeing Copilot delivers highly contextualized content and "next-best-action" care journeys, creating a personalized and real-time engagement experience for every patient.

Another key feature is the Health Risk Assessment (HRA), an NCQA-accredited tool that uses an enhanced question set to reduce completion time, generating a 59% completion rate. This helps quickly guide members to the right care at the right time. Pager’s HRA can also be customized for other data-gathering purposes, such as identifying specific health risks in high-risk populations to improve overall population health.

Finally, ReallyWell includes a dynamic wellness score for each member, displayed in an intuitive visual format. The score updates in real time based on actions taken by members, helping them track their progress and stay engaged in their health journey.
